🧠 Recovery Planning & Goal Support


  • Assist participant to identify recovery goals and priorities
     
  • Develop and review recovery plans aligned to NDIS goals
     
  • Break goals into realistic, achievable steps
     
  • Monitor progress and adjust strategies as needed
     
  • Encourage self-direction and informed choice
     

🤝 Coordination & Service Navigation

  • Help participant understand their NDIS plan and supports
     
  • Connect participant with services (mental health, housing, health, community programs)
     
  • Coordinate between providers to ensure supports work together
     
  • Support communication between participant, family, and professionals
     
  • Advocate for participant needs when required
     

🧩 Capacity Building & Skill Development

  • Build skills in daily structure, routines, and organization
     
  • Support development of decision-making and problem-solving skills
     
  • Assist participant to manage appointments and responsibilities
     
  • Encourage independence in accessing services and supports
     
  • Support confidence in community participation
     

🌱 Recovery & Wellbeing Support

  • Promote hope, motivation, and recovery-oriented thinking
     
  • Support emotional regulation strategies and coping skills
     
  • Assist participant to identify early warning signs of relapse
     
  • Help create crisis or safety plans where appropriate
     
  • Encourage engagement in meaningful activities and community life
     

📋 Monitoring, Reporting & Compliance

  • Maintain accurate case notes and progress records
     
  • Provide reports for plan reviews and funding reassessments
     
  • Monitor risks and communicate concerns to relevant parties
     
  • Ensure supports remain aligned with participant goals and funding
     
  • Follow NDIS Practice Standards and duty of care


📋 Psychosocial Recovery Coaching focuses on helping participants build structure, independence, and long-term stability in their daily lives.


Recovery Coaches work alongside existing services, supporting participants to set goals, develop routines, strengthen community connections, and improve confidence in managing everyday challenges.


 

🚫 What Recovery Coaching is NOT


Psychosocial Recovery Coaching is not clinical therapy, counselling, or crisis intervention.


Recovery Coaches do not replace psychologists, psychiatrists, or medical supports.  

Instead, they work alongside existing services to help participants build structure, confidence, and everyday stability.


Recovery Coaching is also not just “support work”.  

It focuses on long-term goals, independence, and helping participants reconnect with community and daily life.

🧍 Daily Routines & Personal Organization

We support participants to:

  • Get ready for the day or settle into evening routines
     
  • Choose clothing and maintain personal presentation
     
  • Build structure into mornings, evenings, and daily tasks
     
  • Develop routines that support mental health and stability
     
  • Stay on track with everyday responsibilities
     

💊 Medication & Health Routines

We can assist with:

  • Medication reminders and prompting
     
  • Support using Webster packs or medication systems
     
  • Encouragement to follow health routines
     
  • Noticing and reporting any concerns or changes
     
  • Supporting participants to attend health appointments
     

🍽 Food, Energy & Daily Living

We help participants to:

  • Prepare simple meals or snacks
     
  • Maintain regular eating and drinking routines
     
  • Organize groceries or meal planning
     
  • Build confidence managing daily living tasks
     
  • Create routines that support energy and wellbeing
     

🚶 Safety, Mobility & Home Routines

Supports may include:

  • Assistance moving safely around the home
     
  • Help getting out of the house and ready for the day
     
  • Creating routines that reduce stress and overwhelm
     
  • Supporting organization of personal spaces
     
  • Encouraging safe daily habits and structure
     

🌱 Building Confidence & Independence

Our approach always focuses on:

  • Encouraging people to do what they can themselves
     
  • Building confidence in daily living skills

  • Working at the participant’s pace and comfort level
     

📋 How Our Self-Care Support Works

We focus on practical, low-intensity personal supports that help people manage daily life.

While we don’t provide hands-on showering or high-care personal care, we can help participants build routines and connect with the right providers if those supports are needed.

Our role is to support people to feel capable, organized, and confident in everyday life.


 🚫 This support does not include medical or nursing care.  

Our role is to assist with routines, independence, and practical daily living skills — not clinical treatment.

We help participants to:

  • Plan outings and choose activities that suit their goals

  • Practise travel planning and time management
     
  • Build confidence going out regularly
     

🚗 Support Getting There & Back

We can assist with:

  • Travel training and confidence using transport
     
  • Support attending appointments or activities
     
  • Building independence in getting around the community
     
  • Encouragement to maintain regular participation
     
  • Helping reduce barriers to leaving the house

  • Supporting people to participate in ways that feel safe

This might involve starting with small steps — like a short outing or familiar activity — and gradually building toward more independent participation.
