A little about me I started LH Supports in 2020, I had been working as a psychosocial rehabilitation worker for almost 6 years and absolutely loved my role. I decided to change course when the NDIS was formed as the people we supported were being transferred to the NDIS.
I managed to build up the confidence to seek out the support I needed and became a sub contractor, Hence LH Supports!!! Here it started.
LH supports provided NDIS support as a sub-contractor for one year, working alongside others and recognize the potential the NDIS scheme offered to participants. I started a peer group and was able to reconnect many people who all lost contact after transitioning to the NDIS and reconnect people to friends they had met along their recovery journey. The drive to continue to growing this Role, networking and took over....
So the next chapter started.
Case Managers SA was established in 2021, We grew to nice little group of 6..... juggling activities, appointments and life.
I bought on my first sub-contractor and passed on that torch of what I believe psychosocial support/Case management should look like.
We were out linking in with other community groups, actioning personal goals and really starting to recognize the gaps between services. Our peer group has grown and we have welcomed many old and new faces to our fortnightly social group. My dream was to design a service that I would like if I was in need of support of any kind. So when it came time to the next step and register as a company. Case Managers Australia was born.......
April 2022 we are now a company, we have developed our support style and have evidence of how our Psychosocial Support/ Case Management has delivered the results the NDIS intended too. Our next task is to reach the wider community. We already have people reaching out to us that are not NDIS participants, that are seeking support, we are documenting all these interactions anonymously to broaden our service delivery and community support.
Our Business is structured to provide end to end care. Our service delivery is better than others as our participants know they can rely on us.
We don’t make them feel like a number and wear many hats to ensure unique personal needs are meet.
